Good faith estimate
Section 2799B-6 of the Public Health Service Act states that health care providers are required to provide a “Good Faith Estimate” to clients who are not enrolled in a federal healthcare plan (individuals who are uninsured) or who are not filing claims with their insurance provider (individuals who are self-pay). A Good Faith Estimate outlines expected charges for healthcare services. Your Good Faith Estimate document will include:
Client name, client date of birth, a description of services, service-related CPT code(s), list of good or services reasonably expected in the course of treatment, diagnostic codes, Provider Name, Provider NPI, date of Good Faith Estimate, and location of services, including and whether services are provided in person or online via telehealth.
At any time, clients may request their Good Faith Estimate be updated according to new charges, negotiate the bill, or request to see financial assistance is available via the provider.
Disclaimer A Good Faith Estimate shows the costs of healthcare services and related items that are reasonably expected for your healthcare treatment. The estimate is based on information known and available at the time it was created and does not include any unknown or unexpected costs incurred during treatment. If complications or special circumstances arise, you may be charged more than outlined in your Good Faith Estimate. If this happens, federal law allows you to dispute (appeal) the bill.
